On any given night, there are about 7,000 people who are homeless in Santa Clara County, and approximately 2,000 of those are chronically homeless. This situation is not only tragic, it’s preventable and solvable. Destination: Home has an action plan to move from managing to ending chronic homelessness. Housing FirstThe plan is based on Housing First, a nationally recognized best-practices model that is cost-effective and humane. It’s based on the idea that a person’s first need is stable housing. It’s nearly impossible to address other needs when someone has no permanent place to call home. Permanent housing brings stability and reduces the need for high-cost services typically accessed by the chronically homeless. Our StrategyDestination: Home has three-pronged approach that includes streamlining our public systems that serve the homeless, focusing community-based homeless services on the Housing-First model, and increasing the supply of permanent housing. Destination: Home is a leading partner of the Housing 1000 Campaign in conjunction with the national 100,000 Homes Campaign. Housing 1000 is Santa Clara County’s campaign to house 1,000 of the most vulnerable chronically homeless men and women in our community by 2013.
